Circle Of Prayer Enrollment

To enroll someone in the Circle of Prayer, please fill out the form below. Required fields are indicated by an asterisk (*). All of the information you provide will be kept in strict confidence. After you submit the information, we will add the name(s) you submit to our prayer list and send a card appropriate for the occasion of the prayer request. We'll then send you a confirmation letter and a donation envelope. If you choose to donate, your gift will be used to support SSND ministries and the care of retired sisters.
